ScoreFast Predictive SmartConnect Nudge Engines™ demonstrably reduce patient readmission rates

We are delighted to announce today that ScoreData has won the 2019 NTT DATA Open Innovation Contest for the San Francisco center. Our winning entry “Reducing Hospital Readmission Rates With AI/ML powered Nudge Engines” won the overall award from a field of several outstanding contestants that applied,  and from the final eight that presented at the competition.  

ScoreData’s ScoreFast solution showed how we can reliably predict and reduce readmission rates for patients with our scalable nudge engine that can be used to engage tens of thousands of patients across hospital networks.  This innovative solution was demonstrated on the ScoreData platform using a combination of curated publicly available data-sets integrated with proprietary datasets for a large California healthcare system.

ScoreFast nudges have demonstrated that they can positively influence patient self-administration for medication intake, exercise, and diet adherence.  Non-adherence to post-discharge health maintenance instructions leads to a significant increase in hospital recidivism and readmissions, whereas adherence to medical professional recommended prescriptions leads to a significant decrease in hospital visits and readmission, leading to higher efficiency and reduced costs for hospital administration. 

Hospital Readmission costs in the US, estimated at $100B annually, account for a significant percentage of the Healthcare system losses.  So acute is the problem that the Centers for Medicare and Medicaid Services (CMS) of the Department of Health and Human Services (HHS) will publish hospital readmission rates by early 2020 for US Hospitals.
